When to use this template
Use it for the first consultation with a new legal client — the meeting where you gather facts, identify the matter, surface conflicts and statutes, and decide whether to take the case. For strategy after intake, see Case Strategy. For fee or contract negotiation specifically, see Contract Negotiation.
Fields captured
- Client name — the prospective client or organization.
- Matter type — area of law and a one-line matter description.
- Date of consultation — when the intake happened.
- Key facts — the operative facts the client reported.
- Parties involved — each party, role, and relationship to the matter.
- Client goals — what the client wants to achieve.
- Timeline of events — chronology of the events that gave rise to the matter.
- Documents mentioned — the documents the client referenced.
- Potential issues or claims — what the matter could turn into.
- Statute or deadline concerns — any statutes of limitation or filing deadlines discussed.
- Conflicts or ethics notes — anything that might trigger a conflict check.
- Next steps — concrete tasks with optional owner and due date.
- Fee or engagement notes — how the firm will charge, scope, and engagement-letter next steps.
- Key takeaways — the AI's prioritized summary points.
