When to use this template

Use it for any school-side meeting about a student — parent–teacher conference, IEP meeting, 504 review, counselor check-in, advisor meeting. The structure is balanced to capture strengths and growth areas separately, plus concrete support strategies at school and at home. For a doctor visit, see Medical Appointment. Fields captured

  • Student name — who the meeting is about.
  • Grade / class — current placement.
  • Teacher or staff present — who from the school attended.
  • Parents or guardians present — who from the family attended.
