When to use this template
Use it for a household or family check-in — a regular sit-down where the family covers what's coming up, who is doing what, and what to celebrate or worry about. The structure keeps the warm stuff (celebrations) and the heavy stuff (concerns) visible alongside the practical items (chores, schedules). For a one-on-one conversation with your child, see Parent-Teacher flipped perspective — though that template is written for school conferences. For medical conversations, see Medical Appointment.
Fields captured
- Meeting purpose — why everyone gathered.
- Topics discussed — what came up, beyond the planned agenda.
- Decisions made — choices the family made.
- Action items — concrete tasks with optional owner and due date.
- Schedule / calendar items — pickups, drop-offs, events, appointments.
- Chores / responsibilities — who is doing what.
- Celebrations / good news — wins to recognize (kept distinct from concerns on purpose).
- Concerns raised — worries that need follow-up.
- Open questions — unresolved items.
- Next meeting — when and what.
- Key takeaways — the AI's prioritized summary points.

Exports
PDF, DOCX, and Markdown render the meeting in the canonical family-meeting order: purpose → topics → decisions → schedule → chores → action items → celebrations → concerns → open questions → next meeting. Celebrations and concerns are kept in separate blocks so neither gets lost in a long export.
