When to use this template

Use it for any clinical appointment you want to remember and act on — primary-care visits, specialist consultations, urgent care, telehealth visits, dental or vision appointments. The structure is built for what you need at the end of the visit: what was decided, what to take or do, what to watch for, and when to come back. For other family conversations, see Family Meeting.

Fields captured

Provider / specialty — name and specialty of the clinician seen.
Reason for visit — why the appointment happened.
Symptoms or concerns discussed — what was raised.
Examination / findings — what the exam or test showed.
Diagnosis / assessment — the working diagnosis or assessment.
Medications or treatments — what was prescribed, including name, instructions, and purpose.
Tests ordered — anything ordered for later.
Home-care instructions — what to do at home.
Warning signs — red-flag symptoms to watch for.
Follow-up — what to do and when (the next appointment or trigger).
Patient questions — questions the patient asked (and answers if given).
