When to use this template
Use it for a coaching-style conversation about your goals — a personal goal-setting session, a quarterly life review, a coach or mentor meeting, an accountability conversation with a friend or partner. The structure is built to capture the goals, what's blocking them, the habits and systems that would support them, and who is checking in. For money-specific planning, see Finance & Budget. Fields captured
- Time horizon — what period the goals cover (this quarter, this year, the next 5 years).
- Goals — each goal with category, why it matters, and what success looks like.
- Current obstacles — what's blocking progress.
- Supporting habits or systems — the routines that would help, or that you already have.
- Resources needed — tools, money, people, or time required.
- Milestones — milestone name and target date per checkpoint.
- Accountability — who checks in on progress, and how.
- Action items — concrete tasks with optional owner and due date.
- Encouragement or insights — what was said that should be remembered.
- Key takeaways — the AI's prioritized summary points.
